I recomend going to see the doctor in Kettering if your close to that. May need your hospital records but other than that just set up an appointment. Once you do that you go in and will pay 175 before you see the doctor. But once approved you will get an email and you will pay 50 to the state and BOOM you got your card. I have mine for ptsd and chronic pain and I highly recommend you check it out.
